RN Case Manager
Overview
As a Home Health RN Case Manager, you will be responsible for coordinating and managing the care of patients in their homes. This role requires a registered nurse with strong clinical skills, the ability to work independently, and a focus on delivering high-quality, patient-centered care in a home setting.
Key Responsibilities
- Assessment and Care Planning
- Conduct comprehensive assessments and reassessments of patients’ health needs in their home environments.
- Develop individualized care plans that address the unique requirements of patients receiving care at home.
- Coordination of Home-Based Services
- Coordinate with interdisciplinary teams, including home health aides, LPNs, and therapists to ensure a holistic approach to patient care.
- Schedule and organize home visits, ensuring timely and effective delivery of healthcare services.
- Patient and Family Education
- Educate patients and their families on managing health conditions, medications, and any required lifestyle modifications.
- Provide guidance on recognizing and responding to changes in the patient’s condition.
- Direct Patient Care
- Provide hands on care to patients, including but not limited to wound care, IV infusions, blood draws, central line maintenance, catheter care, ostomy care and education, etc.
- Monitoring and Evaluation
- Regularly assess patient progress and modify care plans as needed.
- Monitor vital signs, medication adherence, and overall well-being during home visits.
- Documentation and Reporting
- Maintain accurate and up-to-date electronic health records.
- Prepare and submit reports documenting patient progress, changes in condition, and any relevant information for interdisciplinary team meetings.
